Reporting guidelines for ASHRAE PDH credits

1. All courses/activities must come to my attention two (2) weeks PRIOR to the expected date. The required materials must include:

a. Agenda with course/activity title, date, location, and entire time frame of program
b. Copy of presentation (if more than one presenter, I must receive a copy from each)

2. Within five (5) business days upon the COMPLETION of the course/activity, I must have a copy of the formal sign-in sheet (e.g. fax, scanned copy & e-mailed, direct mail). The sign-in sheet cannot be a typed excel document. It must show the actual signatures of each attendee. This portion, aside of general policy, is also for auditing purposes.

3. Each Licensee (attendee) must report the Area of Practice hours to the license portal at www.myfloridalicense.com and maintain a copy of these records for a minimum of two renewal cycles or four years for auditing purposes.

PDH Carryover

Hours cannot be carried forward into the next renewal period.

Record Keeping

It is the licensee’s responsibility to maintain sufficient records to demonstrate completion of qualifying professional development hours for at least two licensure cycles (four years). A signed statement at time of renewal will be submitted listing title, date, location, provider, area of practice, and number of PDH credits of all qualifying activities. The board will also use approved provider submitted attendance information for confirmation demonstrate licensee's compliance.
